Terms and Conditions: MicroDental Northwest reserves the right to update and modify the program. Rebate will be a percentage of your monthly lab bill with MicroDental Northwest. Minimum monthly lab bill of $3000 required. Maximum rebate of $500 per scanner. Rebate paid as credit on lab bill only. Accounts must be in good standing to receive rebate. Cannot be combined with any other icroDental Northwest promotions, pricing discounts, or coupons. All credits will be processed the following month based on the previous month lab bill.
